NHS 111

From 1 April 2008 staff can no longer accept verbal requests for prescriptions.  They must be made in writing, either by post, request form, e-mail or on-line.

We now offer late appointments (up to 7.00 pm) each evening at Burntwood Health Centre.  You can book these via EMIS access or at reception. 

TEXT MESSAGE SERVICE: We now offer a text reminder service for appointments and health checks in the near future.   If you would like to use this service please complete a consent form at the practice.



Welcome to the Fulfen Practice                           teddy


We aim to provide a high quality service and all our team try and do so in a compassionate and caring way.  We have a long serving and loyal team of staff which really helps in getting to know you, your family and your needs.

We provide our services from both Burntwood Health Centre at Hudson Drive and Chasetown Medical Centre on Chasetown High Street.  Appointments can be booked at either site depending on your preference - our computer systems are linked so staff at both sites have full access to the information they need. 

We really value our relationship with our patients and have an active patient group to help us.  Details of this can be found via the link on the right hand side of the page. There is a regular newsletter to keep you informed and we update the website regularly to ensure you have the lastest practice news.

As well as the medical service the practices provide, we also have a Minor Illness Service (available at Burntwood Health Centre), Stop Smoking and Counselling Services, details of which are under Clinics & Services tab at the top of this page. 

We hope we can help you to live a full and active life.


Online Services

If you have collected your Access ID form from the surgery please click on the "Order Repeat Prescription" box below to use on-line appointments and prescriptions. The first time you access the site you will need to register (click on link below). 

Unfortunately it is NOT possible to book appointments with the Nurse on-line.  See our Emis Access User Guide for further information.

Parking at the Health Centre

We understand that parking at the Health Centre can be difficult at times but we would like to remind all patients to please be considerate when parking.  Patients are welcome to use the car park attached to the Health Centre but are not entitiled to park in the spaces directly behind the shops at the back of the Health Centre car park.  These spaces are for the use of the residents of the flats above the shops only.  If you park on Hudson Drive please ensure that you do not block any driveways.
